South Korea’s National Health Insurance Service is reviewing plans to assess health insurance premiums on day labor income and certain separately taxed financial and pension income currently excluded from the premium base. The move seeks to align premium obligations with taxable income and reduce disparities between people earning similar amounts through different income types. The NHIS is also pursuing an annual, agency-led income reconciliation system for regionally enrolled subscribers, replacing the current process that generally requires a subscriber’s request. The agency estimates the reform would increase premiums for 3.94 million households by 3.2911 trillion won ($2.24 billion), while providing refunds totaling 1.8014 trillion won to 2.32 million households with reduced income. The projected net annual increase in health insurance finances is 1.4897 trillion won. Specific income categories, thresholds and implementation dates for expanding the premium base have not been decided.
